Your Healthcare Plus

Welcome to the Your Healthcare Plus online site for medical providers.

Your Healthcare Plus is a statewide disease management program designed to support Healthcare and Family Services (HFS) medical providers with the medical management of patients and to improve the quality of care and life for the following Illinois healthcare program enrollees:

Disabled adults with chronic illnesses including asthma, coronary artery disease, chronic obstructive pulmonary disease, diabetes, and heart failure; complex illnesses such as hemophilia, HIV, ESRD and cancer; and co-occurring conditions including depression, schizophrenia and chronic pain
Children and adults with persistent asthma
High frequency emergency room users

HFS has commissioned McKesson Health Solutions to administer Your Healthcare Plus. The focus of the program is on promoting and sustaining the patient-physician relationship. The program is designed to work with providers to compliment existing disease and case management programs already in place. Your Healthcare Plus benefits medical providers through the added support of:

Registered nurses
Pharmacists
Behavioral health specialists
Nurse case managers
Social workers

Your Healthcare Plus staff members live and work in the communities they serve. Each locally based team serves as a resource and support for patients, their primary care physicians and other providers of health services involved in their care. Some Your Healthcare Plus staff will be located in high volume outpatient offices, hospitals and long-term care institutions to facilitate communication and coordination. Based on risk level, patients may receive nurse education and counseling by either phone or community based contact. Interventions are tailed to a patient's risk level and social, language and cognitive needs. All patients and providers are offered written educational materials and can access additional information and assistance via the web.

Your Healthcare Plus staff help patients:

Find and use a medical home
Get education and coaching support to understand and adhere to their doctor's plan of care.
Obtain the appropriate medical care needed to reduce their health risks.

Your Healthcare Plus is designed to:

Improve the health and wellness of participants
Reduce avoidable inpatient and emergency room use
Increase the use of a medical home
Promote adherence to drug regimens that are cost and clinically effective
Increase the number of patients who receive care consistent with evidence-based practice guidelines
Provide technology and office support that allows better communication across all team members
Reduce costs.
